Terms and Conditions
Appointments bookings
We are encouraging as many remote (telephone/skype/zoom) consultations as possible during these uncertain times. However, we also recognise that it will be necessary for some appointments to be face to face. The following terms and conditions have been updated to meet new Covid-19 requirements:

All patients will receive a Covid-19 questionnaire at the time of booking.

New patients will also receive a Patient History Questionnaire, Consent and GDPR forms to sign.

An appointment reminder will be emailed to you 5 days before your appointment.

All questionnaires are sent and received securely via Cliniko.
Day of appointment

For telephone consultations, you will be asked to call the clinic on 020 7486 2196 at the scheduled time.

For face to face appointments, you should report to the receptionist who will direct you to the waiting room. The receptionist will let us know you have arrived and when we are ready to see you, a member of WMC staff will greet you at the bottom of the stairs.

Our staff will be wearing masks for all face to face appointments, and visors will be worn for some treatment and testing sessions.

We encourage all patients to wear a mask. We are happy to provide you with one if you don’t have your own.

Additional hand sanitiser is available in all consulting rooms and at WMC reception, which we encourage you to use.

Additional cleaning of contact areas will be carried out between patients.

You will be able to purchase products at your visit as normal, but you will also have the option to have them delivered to your home if you prefer.

Due to social distancing and capacity numbers, we discourage you from bringing a friend or family member with you to your appointment. However if you need a chaperone for mobility reasons, please let us know at the time of booking so that we can make special arrangements.
We do our best not to keep our patients waiting, and request that you keep to the scheduled appointment time to ensure adequate time for your session.
Seeing other practitioners
Please inform WMC if you are seeing another practitioner. WMC reserves the right not to offer treatments to patients undergoing treatment by another practitioner.
Children
We only see children over the age of 5 years who must be accompanied by an adult at all times.
Privacy and confidentiality
We endeavour at all times to respect the privacy and dignity of all our patients and have appropriate procedures and policies in place regarding patient information and data protection. Your treatment and care is confidential and no information shall be passed to a third party without your written consent.

Repeat prescriptions
For repeat prescriptions to be authorised by Dr Daya you will need a follow up appointment at least every 6 months to review your medication and progress.
Mobility
If you have any mobility problems please notify us in advance so we can ensure you receive help in entering the building.
Costs and payment terms

Payment will be taken for all tests at the time of booking. Consultations or supplements recommended will be charged separately.

We accept all payment cards except American Express.

We no longer accept payments by cheque or cash.

Patients with Medical Insurance must make payments in full to the Wholistic Medical Centre and then make arrangements to reclaim this from their insurer.
Cancellation

Where it is necessary to cancel we request 48 hours’ notice, and will reschedule your appointment.

No shows or cancellations within 48 hours of appointment will not be eligible for a refund.
Refunds

Services received are not refundable.

Where it is not possible to reschedule a prepaid booked appointment, a refund can be arranged providing at least 48 hours’ notice is given.

We offer a full refund on unopened products if returned within 14 days from date of purchase.
