| Please make sure you give us full details including your telephone contact numbers. All consultations should be booked through Dr Daya's secretary between 10am and 6pm Monday Friday on an appointment basis by telephoning the above number. We recommend that you have your initial appointment with Dr Daya when she will formulate your assessment and treatment plan with you. |

| Should you require further information we can send you our information pack, containing general information as well as any specific information you request. If Dr Daya's secretary is unable to help an appropriate member of our team will call you back to advise or talk you through any specific queries. You may wish to save time by completing a questionnaire before your first visit. |

| Children under 16 must be accompanied at all times by a responsible adult. If you have any mobility problems, please notify us in advance of your visit so that we can ensure that you receive help in entering the building. We ask you give us at least 24 hours notice of cancellation otherwise a 50% cancellation fee may be charged. |
